Medicare GUIDE Program
GUIDE Medicare Benefit Program is available for those living with dementia & their unpaid caregivers. This benefit of up to $2,563.00
seeks to provide support at every step of the disease progression. Those enrolled are assigned a Dubin Center Care Navigator who can provide support, connections to resources, and education. Enrollees’ caregivers are also eligible for a yearly stipend to help pay for respite services. The Dubin Center is an official GUIDE Partner Organization with Healthspan Partners of SWFL.
Participants must have Traditional Medicare and a diagnosis of a dementia-related illness. Individuals cannot be enrolled in an Advantage plan, in hospice or PACE programs. Eligible participants may live at home, in independent living, assisted living, or memory care communities, but not in a skilled nursing facility.
