Medicare GUIDE Program

The Guiding an Improved Dementia Experience (GUIDE) Model is a new benefit program from the Centers for Medicare & Medicaid Services (CMS), that provides up to $2,500 annually for respite services, giving caregivers much-needed relief and helping delay or prevent long-term care placement. It also enhances dementia care by offering care navigation services, structured care planning, medication management, and caregiver support. By integrating these services into Medicare, GUIDE aims to improve quality of life, reduce hospitalizations, and ease the burden on families.

Eligibility is limited to individuals with Traditional Medicare and a dementia diagnosis. Those enrolled in Medicare Advantage, Hospice, or PACE are not eligible at this time.

The Dubin Center has been approved to provide care navigation under the GUIDE program and is a member of the Healthspan Partners of SWFL network. Please visit https://healthspanswfl.com/ for more information or call The Dubin Center to enroll: 239-437-3007.
